Most software projects fail before the first line of code is written.

The failure happens in the brief. Vague requirements, assumed context, unstated constraints and unrealistic timelines are baked into the project before any engineer sees it. Then the team builds what was asked for rather than what was needed — and the gap only becomes visible in production.
